VO₂max — maximal oxygen uptake — is the best single measure of aerobic fitness: the maximum amount of oxygen your body can use per minute during intense exercise. It captures how well your heart, lungs and muscles transport and consume oxygen, which is why endurance athletes and fitness enthusiasts both track it closely.
What the number really means
Measured in millilitres of oxygen per kilogram of body weight per minute (mL/kg/min), VO₂max describes how much fuel your body can burn aerobically at maximum effort. A higher score means your "engine" is bigger. It is influenced by your genetics, your training history and your size — carrying less excess weight makes the same heart and lungs perform more efficiently on a per-kilogram basis, which is one reason lighter, fitter people tend to score higher.
The resting-heart-rate method
There is a strong link between a low resting heart rate and high fitness. The common non-exercise estimate is:
VO₂max ≈ 15.3 × (max HR ÷ resting HR)
With max HR = 220 − age. For a 35-year-old with a 65 bpm resting HR:
max HR = 185 → VO₂max = 15.3 × (185 ÷ 65) ≈ 43.5 mL/kg/min
The logic runs both ways: a low resting heart rate suggests an efficient, well-conditioned heart, which tends to accompany a higher VO₂max. The 15.3 coefficient is an empirical constant drawn from studies correlating resting heart rate with measured aerobic capacity — a useful shortcut, but one that treats everyone through the same lens.

More accurate methods
Field tests like the Rockport one-mile walk, Cooper 12-minute run or a Ramp/Bruce treadmill test are more accurate, and lab gas analysis is the gold standard. The resting-heart-rate estimate here is the quickest and roughest option: it needs no exercise at all, but it inherits all the imprecision of the 220 − age max-HR guess and the day-to-day variability of a single resting-pulse reading. For trend-tracking, measure your resting heart rate on waking, before coffee or movement, and use the same conditions each time.
